Admissions

Candidates, or their representatives, may file an application for admission to our facility.

Referral of Admissions:

bulletAll referrals for admissions must be made through the Social Services Department.
bulletAll required paperwork must be completed and submitted to the Social Services Department.
bulletReferrals will be screened by the Admissions Team to determine appropriateness of placement.
bulletAfter approval by the Admissions Team, referrals will be placed on a waiting list until an appropriate bed is available.
bulletReferring agency/person will be notified if patient does not meet admission criteria.

Pre Admission Requisites:

bulletPersonal Interview and Application/Social History forms to be completed.  Once completed and returned, the name will be placed on our waiting list.
bulletPatient Care Referral form must be completed by the applicant's attending physician.  Approval for intermediate care or if Medicaid eligible, must be obtained through the Office of Long Term Care, Concord, New Hampshire.

Financial Review:

Medicaid applicants financial eligibility will be determined by the Department of Health and Human Services, Littleton District Office.

Private paying applicants will be admitted to to the Grafton County Nursing Home at the current per diem for room and board.  This does not include medication or physician charges.

Medications:

Medications are obtained through Unicare Health Services, Inc. Londonderry, New Hampshire.  For private paying residents, the responsible party of the resident will be billed for medications directly from Unicare.

Advance Directives:

Prior to admission, the applicant must have established a Durable Power Of Attorney (DPOA) for healthcare and finances or guardianship.

Request for Admission Forms:

Upon request, pre-admission forms will be mailed to the person/agency.  It will be their responsibility to have the forms completed and returned to the Social Services Department.

Referrals cannot be processed until all necessary forms are completed.
